Immunization Record: This form should be completed by your family physician and must be completed for first time campers regardless of the program.

Health History: Must be completed by parent/guardian and sent in every year the child is a camper.

Additional Information for the Counselor: Must be completed by the parent/guardian for campers participating in all core, challenge, or advanced programs.
